Request access to my medical records?

Please complete a subject access request form for a copy of your medical records – these forms are available at reception or can be requested via email at gmicb-sto.p88011.receptionteam@nhs.net

 

 

 

Register for online record access?

Please complete a form requesting online access to your records – this can be done in person via reception or via email  

gmicb-sto.p88011.receptionteam@nhs.net 

Your request will be reviewed by a GP – your records can then be viewed via the NHS app or Patient Access
