How do I...Register with the practice?

Registration forms can be completed in person at either of our practice sites, the registration form is available here.

Pre-registration forms can be filled in online – patients can then attend in person to confirm that the details are correct. 

Patients are also asked to complete the medical questionnaire when they register – this can be done in person, or through an online here.

How do I...Change my address/phone number etc?

Patients can do this online via a form here.

 This can also be done by emailing the practice gmicb-sto.p88011.receptionteam@nhs.net  

Patients can also do this in person via reception.  

Please note patients may be asked for evidence of change of address. 

Patients who have moved out of our practice boundary will be asked to re-register with a local GP.
